Pedicure Consent Form

Welcome to Polished on Pleasant!

According to state law in Massachusetts, Prohibition 6.2:

No manicurist, demonstrator, instructor, or manicuring student shall provide services to persons whose hands, feet, or nails are afflicted with boils, infected wounds, open sores, abrasions, weeping dermatological lesions, impetigo, pediculosis, or fungal infection.

 

If your nails are white, black, gray, or yellow in color, flaking, peeling, or thick, you may be afflicted with the above-mentioned conditions. We ask that you contact a podiatrist before booking an appointment and bring in a doctor's note to receive a service.

In compliance with the above law, please complete this form before your appointment. If you answer yes to any of the above-mentioned conditions, you MUST call and cancel your appointment with 24 hours notice, or our cancellation policy will apply.

 

If you arrive for your appointment with any of the above-stated conditions, your service will be denied, and you will be charged our cancellation policy. By booking your appointment, you agree to these terms.

Also, this is a reminder that we no longer offer gel pedicures and do not remove gel from toenails.

Cancellation Policy

When you commit to a service with us at Polished On Pleasant you agree to the following cancellation policy: If you must cancel or reschedule your appointment, we ask that you notify us at least 24 hours in advance to avoid being charged. Cancellations made under that window are subject to a fee of 50% of the value of your service. If you miss your appointment completely with no advanced notice, a 100% fee of all services booked will be charged. All charges listed above will be applied to your card on file, which is required to book an appointment.
